Case Management

Case ManagerCase managers at the Lighthouse of Broward assist the client from the time of their referral until the time of their discharge in all aspects of vision rehabilitation services. Clients are referred from various sources such as self-referral, family, friends, eye care providers, the Division of Blind Services, other social services agencies and health care providers. The case manager is responsible for doing the initial assessment and obtaining all necessary documents to participate in the Lighthouse of Broward programs. The case managers are responsible for making sure the client is getting the training provided as outlined in their rehabilitation plan. They are also responsible for providing the clients with referral resources and assistance as needed. There are case managers assigned to each of the following programs: The Vital Living Program which serves adult clients who are learning independent skills for the purpose of improving their quality of life; The Ready to Work Program which serves clients whose goal is to return to the work force or maintain their current employment; and the Youth Program which serves clients from birth until high school graduation in learning the skills necessary to be independent and productive. Case Managers are the liaison for the client with the rehabilitation instructors, the Division of Blind Services, and other community resources.
